The Origins of The MATCH

The National Resident Matching Program (NRMP), commonly referred to as "The MATCH," was established in 1952 to create a fair and efficient system for assigning residency positions to graduating medical students. Prior to its formation, medical students had to navigate a chaotic and competitive job market, often receiving residency offers as early as their third year of medical school, leaving little time for informed decisions. The introduction of The MATCH standardized the process, ensuring that both applicants and residency programs could make more equitable and well-informed choices.

The system was modeled after an algorithm developed by economists Alvin Roth and Lloyd Shapley, who later received a Nobel Prize for their work on matching theory. This algorithm optimally pairs applicants with residency programs based on ranked preferences from both parties, leading to a process that is largely fair and efficient.

How The MATCH Works

The MATCH process begins long before MATCH Day, requiring months (or even years) of preparation from medical students. Here’s a step-by-step breakdown of the journey:

1. Preparing for The MATCH

Students usually begin preparing for The MATCH well in advance, ensuring they meet the necessary qualifications and build a strong application. Key elements include:

  • USMLE Exams: Passing the United States Medical Licensing Examination (USMLE) Step 1 and Step 2 is a critical requirement for most residency programs.
  • Clinical Rotations & Experience: SJSM’s extensive clinical rotation program provides hands-on experience in U.S. hospitals, an advantage for students applying to competitive residency programs.
  • Letters of Recommendation (LoRs): Strong LoRs from faculty, preceptors, or attending physicians play a crucial role in demonstrating an applicant’s readiness for residency.
  • Personal Statement & CV: A well-crafted personal statement and professional CV help showcase an applicant’s qualifications, aspirations, and reasons for choosing a particular specialty.

2. The Application Process

In September, students submit their applications through the Electronic Residency Application Service (ERAS). ERAS is an online platform that streamlines the application process by allowing students to submit their documents, personal statements, LoRs, and other materials to residency programs across the country.

3. Interview Season

From October to January, residency programs invite selected applicants for interviews. These interviews are an opportunity for both the applicant and the program to determine if they are a good fit for each other. Due to the impact of the COVID-19 pandemic, many residency interviews have shifted to a virtual format, making adaptability and professionalism even more critical.

4. Ranking & The Algorithm

Following the interview season, both applicants and residency programs submit their ranked preferences:

  • Applicants rank the programs they interviewed with based on their personal preferences.
  • Residency programs rank applicants they would like to accept into their program.

The NRMP algorithm then processes these rankings to determine the best possible MATCH for each applicant. The system is designed to prioritize the applicant's choices whenever possible, creating optimal pairings between students and programs.

5. MATCH Week & MATCH Day

MATCH Week takes place in mid-March and is filled with anticipation and emotion for thousands of medical students worldwide. The week unfolds as follows:

  • Monday: Applicants find out whether they have matched but not where.
  • Tuesday to Thursday: The Supplemental Offer and Acceptance Program (SOAP) takes place, giving unmatched applicants an opportunity to secure a residency spot in programs with unfilled positions.
  • Friday - MATCH Day: The long-awaited moment arrives when students receive their official MATCH results, revealing where they will spend the next few years training as resident physicians.
