Referral Form

I understand that I will only receive the referral credit if I refer someone before they complete the formal application with the school. All referral applications are subject to verification by the school's official. A maximum of 4 referrals will be processed toward a Basic Science semester. A maximum of 2 referrals will be processed towards a Clinical Science semester. Excess referrals will be rolled forward to a future semester.
